Criticism of demarcation between doctor and nurse in the health system

Criticizing some common demarcations in the health system, the head of the nursing system organization said: using the literature of "doctor and non-doctor" or creating separation between working groups is not compatible with the team nature of health services.

According to Ambassador Arjan Ahmad Najadian, he added: Doctors, nurses, midwives, health experts, laboratory staff, imaging and support forces, each have a role in providing services and should be considered part of a coherent group within the framework of their responsibilities and expertise.

Emphasizing that unity in the health system does not mean ignoring professional differences, he said: each group has its own working conditions and demands; But besides these demands, there are also common issues that require continuous cooperation and dialogue to pursue them. We must not allow unnecessary demarcations to undermine professional solidarity among health workers.

Referring to the living conditions of health workers, Nejatian said: the country's economic conditions and limited resources should be taken into account, but payment policies should also be adjusted in such a way that, while paying attention to the responsibilities, expertise and working conditions of different groups, they do not create a feeling of discrimination and injustice among the employees.

Referring to the recent changes in some payment regulations and health service tariffs, he added: it is necessary to improve the living conditions of faculty members and pay attention to their recruitment and retention problems; But at the same time, the status of other employees of the health system should also be taken into consideration. One cannot see the livelihood issues of one group and remain oblivious to the problems of other groups.

The head of the nursing system organization said: the effects of the increase in payments are not the same for all groups and it is necessary for policymakers to consider the actual amount received and the working conditions of the employees in addition to the percentage increase. Justice in payment requires simultaneous attention to professional responsibility, level of education, workload, difficulty of service conditions and the contribution of each group in providing health services.

He also pointed to the problems of nurses working in round-the-clock comprehensive health centers, and emphasized the need to pay attention to working conditions and the implementation of laws related to employees and said: some of these centers actually shoulder part of the burden of medical services in the regions; Therefore, the situation of manpower, receipts and the implementation of regulations related to their employees should be examined according to the working realities.

Referring to the difficulty of attracting and retaining nurses in Tehran and some regions of the country, Nejatian considered welfare and livelihood support as one of the effective solutions to reduce the economic pressure of employees and added: Measures such as the development of welfare facilities and the provision of support packages can help the health system's longevity. We expect that the successful experiences at the Ministry of Health headquarters level will be taken into consideration by the officials of medical sciences universities and that employees in different regions will receive appropriate support.
